2017 Registration

Registration for Lincoln Water Polo is now open!  Follow this link for an online registration form and instructions:

 

2017 LHS Water Polo Club Athlete Registration Form

 

In addition to the online form and waiver, please complete the following Lincoln forms:

• Lincoln Code of Conduct form

• Physical Exam form OR Medical History Update form

NOTE: A Physical Exam Form must be submitted every other year.  The Medical History Update should be submitted in the year that the Physical Exam Form is not required. 

• Registration Fee (Please make checks payable to Lincoln Water Polo.)
  • $335.00  
  • $360.00 (after July 15, 2017)
  • $35.00 (Federal Free/Reduced Lunch Recipients Only)

Water polo practice will start on Monday, August 14, 2017.  Complete forms and payment must be received prior to the first practice.
