2018 Registration

Registration for Lincoln Water Polo is now open!  Follow this link for an online registration form and instructions:

 

2018 LHS Water Polo Club Athlete Registration Form

 

In addition to the online registration, please complete the following Lincoln forms (you'll receive an email when you complete the online registration with these instructions):

• Lincoln Code of Conduct form
• Physical Exam form OR Medical History Update form
  (NOTE: A Physical Exam Form must be submitted every other year.  The Medical History Update should be submitted in the year that the Physical Exam Form is not required. 
• Registration fee (Please make checks payable to Lincoln Water Polo.)
• $335.00  
• $360.00 (after July 15, 2018)
• $35.00 (Federal Free/Reduced Lunch Recipients Only) 

Pre-season water polo practice will start on Sunday, August 12, 2018.  Complete forms and payment must be received prior to the first practice.
