Registration Information for 2013 LHS Water Polo Club


Hi LHS Water Polo Families,

Please read and follow all deadlines for returning completed paperwork. The first deadline is July 15th.

If your parents are not receiving the emails,  please contact lhswaterpolo@gmail.com ASAP. Include your parents contact phone and email.

This email contains super important information in advance to help you plan ahead for the upcoming 2013 Water Polo season which will begin Monday, August 19th.

Follow these steps/forms carefully! 

Please complete all documents and send along with the registration fee a check for $325 before July 15th (if your athlete is required to have a physical by a doctor please do so by Aug 1st).

All athletes must complete new paperwork, including RETURNING athletes.

1. Please complete the attached registration/liability form for the 2013 season. The name on the registration form must be exactly the same as your School ID card ~ no exceptions.   The cost for the season is $325. If your form and payment are not received by July 15th, the cost will be $350 (no exceptions). Make your $325 checks payable to "LHS Water Polo Club". Participation in a club sport such as water polo does not count toward Lincoln's maximum athletic participation fees. If your student also plans to participate in a school team winter and spring sport, you will need to pay for those separately.

The team has some funds set aside for scholarships to help families who need assistance. In general, scholarship assistance is available for families who qualify for the federal  free/reduced lunch program  but may also be made to families whose current circumstances require some financial help this season. To request a scholarship, please reply to lhswaterpolo@gmail.comScholarship requests/recipient information is kept confidential. 2.  ALL athletes must compete the attached updated clearance form for 2013 (including returning).   Please complete and mail a copy to Anita by July 15th. Just write in the year 2013.

3. Complete the attached Mac Waiver and Rules of Conduct.   Please complete and mail a copy to Anita by July 15th. The name on the MAC Waiver form must be exactly the same as your School ID card ~ no exceptions.

4. LHS Club waterpolo athletes must have an approved physical exam and history completed by a qualified medical provider before participating. This form is due by August 1st.   The physical must be done upon entering 9th grade and upon entering 11th grade. Sometimes there are free clinics at providence and legacy.  Please complete and mail a copy to Anita by August 1st

5. Version 1.0 of the 2013 LHS Water Polo Pre-season and Season schedule is attached.  All practices are at PSU and the MAC Club.  The schedule is subject to change. 

6.  Parent/Team Meeting ~ We will have a meeting during the 2nd week of pre-season. Details to follow. It will be a nice social gathering. We will cover Water Polo 101.

7. Quickly gather information at http://lincolnpolo.blogspot.com/ but other good sources are via email, talking with teammates, coaches and captains and the facebook page  http://www.facebook.com/LincolnHighSchoolWaterPolo?ref=hl

8. We will send out apparel information within the next 4 weeks. Please add your info to the roster today so you receive all emails. lhswaterpolo@gmail.com

Code of Conduct (attached)
Updated Physical Clearance (attached) all athletes must complete and must be signed by a parent or guardian (write in 2013 at the top

If necessary, send by August 1st
Approved physical exam and history completed by a qualified medical provider (if entering 9th or 11th grade) good for 2 years(just write 2013 on top)

And we have 3 car wash fundraisers ~ June 22nd, July 13th, August 17th.  Everyone must participate (except for those in THills tournaments those weekends).  Annie Shephard will be confirming times within the next week.  Generally from 11-2.  Location at the Shephard Chiropractic Clinic Parking Lot, 2323 NW Flanders!

This is a lot of information to digest ~ we have made every attempt to provide you with all the information we know at the point.  And we wanted you to have it early this year.
