Registration for Lincoln Water Polo is now open! Follow this link for an online registration form and instructions:
Lincoln High School Water Polo Club – 2016 Registration Form
In addition to the online form and waiver, please complete the following Lincoln forms, available on the Lincoln Athletics registration page:
1) Lincoln Code of Conduct;
2) A Physical Exam Form no older than two years old (typically 9th and 11th grades) OR a Medical History Update in alternate years (typically 10th and 12th)
Please send completed materials with a registration fee of $325 before July 15, or $350 after July 15, to:

Do not turn forms in to the Lincoln Athletic Department - as a club sport we are responsible for registration.
For students on Federal free/reduced lunch, the registration fee is $35.
